Ao acessar e utilizar os serviços da MYA CLÍNICA, o paciente declara estar ciente e de acordo com as condições abaixo descritas, concedendo seu consentimento livre, informado e inequívoco:

O paciente poderá autorizar ou não a MYA CLÍNICA, seus sócios, colaboradores e contratados, a captar, armazenar, tratar e utilizar imagens, fotografias, vídeos e/ou gravações realizadas antes, durante e/ou após a execução de procedimentos estéticos. Essa utilização será exclusivamente para fins de divulgação em mídias sociais, sites institucionais, materiais publicitários e/ou científicos, sempre em conformidade com a legislação vigente sobre direitos de imagem e privacidade.
1.2. O paciente está ciente de que, uma vez divulgada sua imagem em redes sociais ou plataformas digitais, pode não ser possível garantir a remoção completa do conteúdo devido à natureza da internet.
2.1. O paciente reconhece que, no âmbito da medicina e da estética, não existem garantias absolutas quanto ao resultado final dos procedimentos, uma vez que as respostas individuais podem variar de acordo com fatores próprios de cada organismo.
2.2. O paciente declara ter sido informado sobre os riscos, benefícios, alternativas e limitações dos tratamentos propostos, bem como ter tido oportunidade de esclarecer todas as dúvidas previamente.
3.1. A MYA CLÍNICA não realiza devolução de valores pagos por procedimentos já realizados ou contratados, independentemente do grau de satisfação com os resultados, salvo nos casos previstos em lei.
3.2. Essa política está em conformidade com o Código de Defesa do Consumidor e demais normas aplicáveis.
4.1. A MYA CLÍNICA possui todas as licenças e autorizações necessárias ao seu funcionamento regular, incluindo Licença de Funcionamento nº 26007/2025 e Registros nº E178307, E178306, E178305.
4.2. Tais documentos encontram-se disponíveis para consulta nas dependências da clínica.
5.1. O paciente declara ter lido integralmente este termo, compreendido seu conteúdo e estar de acordo com todas as condições aqui estabelecidas.
By accessing and using the services of MYA CLINIC, the patient acknowledges and agrees to the conditions described below, granting their free, informed, and unequivocal consent:

The patient may authorize or not authorize MYA CLINIC, its partners, collaborators, and contractors to capture, store, process, and use images, photographs, videos, and/or recordings taken before, during, and/or after the execution of aesthetic procedures. Such use will be exclusively for dissemination on social media, institutional websites, advertising, and/or scientific materials, always in compliance with applicable laws regarding image rights and privacy.
1.2. The patient acknowledges that, once their image is published on social networks or digital platforms, it may not be possible to guarantee its complete removal due to the nature of the internet.
2.1. The patient acknowledges that, in the field of medicine and aesthetics, there are no absolute guarantees regarding the final results of procedures, as individual responses may vary according to factors inherent to each organism.
2.2. The patient declares that they have been informed about the risks, benefits, alternatives, and limitations of the proposed treatments, as well as having had the opportunity to clarify all questions in advance.
3.1. MYA CLINIC does not provide refunds for procedures already performed or contracted, regardless of the degree of satisfaction with the results, except in cases expressly provided for by law.
3.2. This policy is in compliance with the Consumer Protection Code and other applicable regulations.
4.1. MYA CLINIC holds all necessary licenses and authorizations for its regular operation, including Operating License nº 26007/2025 and Registrations nº E178307, E178306, E178305.
4.2. These documents are available for consultation at the clinic’s premises.
5.1. The patient declares that they have read this form in full, understood its content, and agree with all the conditions established herein.
